READ CAREFULLY: ALL registrations must be completed in full. If not completed, payment will not be accepted. No toys, radios, cell phones, clothing or other valuables are allowed. The organization is not responsible for the loss or theft of personal items. On the first day of training we will require the receipt of payment indicating that your balance is in balance; otherwise, unfortunately, we will not be able to accept the participant. NON-REFUNDABLE MONEY - neither the deposit, nor partial or full payment are refundable. AUDIOVISUAL AUTHORIZATION: by signing this registration form I authorize Dream Chaser Basketball LLC , for the use of video footage and/or photographs during the camp. I understand that the use of the image or voice, will be primarily for teaching or promotional purposes of the organization, in any media possible, (press, radio, television, Internet, etc.). If the participant causes damage to property, facilities or equipment, he/she will be responsible for the replacement or payment of the same. If the participant incurs in serious disciplinary offenses, repeated misbehavior, disrespect to the authority of Dream Chaser Basketball or fellow participants and/or any other inappropriate behavior, he/she will be suspended from the training without refund of money. Enrollment in the training implies full acceptance of the above conditions. I acknowledge that participation in DCB-SUMMER BOOTCAMP 2023 is on a voluntary basis and release Dream Chaser Basketball, LLC, its directors, officers, agents, organizers and sponsors from all liability in the event of an accident or physical injury to my child, whether incidental or arising out of his/her participation in the event. I authorize my child to participate in DCB-SUMMER BOOTCAMP 2023. I understand and agree to the statements presented in this document.
